Social and Emotional Wellbeing Outreach Worker

Key responsibilities:

  • Work with the client and their family as well as service providers to provide culturally sensitive strengthening plans
  • Outreach work with clients and assessing and assisting with immediate needs
  • Addressing needs and causes of underlying mental health challenges
  • Strengthen Community members connection to culture, land and Community
  • Supporting client for the duration of a treatment plan
  • Supporting clients during arrested referral stages
  • Assist clients with any education and training requirements
  • Attend case management meetings for client care
  • Case coordination of care teams and participate in information sharing (MARAM)
  • Support clients in any necessary aftercare programs
  • Carry out and design groups and programs to support the clients connection to community and culture

Successful candidates should have:

  • Experience working with Aboriginal communities
  • Certificate IV in aboriginal and/ Torres Strait Islander Health Care or tertiary qualification in health, social and behavioral science
  • Right to work in Australia
  • A valid driving license
  • Working with Children check or ability to obtain
  • Police Check or ability to obtain 
  • Current government mandated vaccinations
