Social and Emotional Wellbeing Outreach Worker

Key responsibilities:

Work with the client and their family as well as service providers to provide culturally sensitive strengthening plans
Outreach work with clients and assessing and assisting with immediate needs
Addressing needs and causes of underlying mental health challenges
Strengthen Community members connection to culture, land and Community
Supporting client for the duration of a treatment plan
Supporting clients during arrested referral stages
Assist clients with any education and training requirements
Attend case management meetings for client care
Case coordination of care teams and participate in information sharing (MARAM)
Support clients in any necessary aftercare programs
Carry out and design groups and programs to support the clients connection to community and culture

Successful candidates should have:

Experience working with Aboriginal communities
Certificate IV in aboriginal and/ Torres Strait Islander Health Care or tertiary qualification in health, social and behavioral science
Right to work in Australia
A valid driving license
Working with Children check or ability to obtain
Police Check or ability to obtain
Current government mandated vaccinations
