The Home Stretch Program supports young people leaving out-of-home / foster care with one-on-one coaching and other support, from when they turn 18 and until the age of 21.  The Home Stretch WA program was designed with young people with lived experience of having left care and made their way to independent adulthood, often with little, if any help.

As an Aboriginal Community Controlled Organisation (ACCO), the Home Stretch service focuses on the needs of young Aboriginal people.  Some of the challenges they face include homelessness, lack of independent living skills, responsibilities for other family members, poor education and employment options, early pregnancy and the risk of having their child taken into care, involvement with the justice system, health issues, and substance abuse resulting from trauma and other issues.

As a transition coach you will work with at risk Aboriginal young people and support them to overcome their challenges, meet their needs, reach their goals, and create the life they want to live.

About the Organisation 

Marra Worra Worra Aboriginal Corporation is the largest and oldest resource agency delivering services to people and communities across the broader Fitzroy Valley area of the Kimberley Region of Western Australia. The primary aim of Marra Worra Worra is to work with Bunuba, Gooniyandi, Wangkatjungka, Walmajarri and Nyikina groups in the Fitzroy Valley to strengthen culture, support community and create sustainable employment solutions. 

We are seeking an experienced and skilled person with a significant background in the child, youth and family sector. Your approach will be trauma informed, strength based, person centred and culturally secure. You will ensure the right to self-determination drives your practice, while promoting the wellbeing of young people. 

About the Role 

As a Home Stretch Transition Coach, you will provide 1:1 professional support, guidance, and case coordination to young people transitioning from Out-of-Home Care, offering them an extension of support and resources to the age of 21 years. This service delivery will be on an outreach basis so an ability to work flexibly is essential.

This will include:

  • Regular engagement and communication with young people to assist support their transition to independence. 
  • Developing a young persons’ natural networks and assisting them to build and maintain connection to culture, country, family and community supports. 
  • Offering and brokering access to a safety net of financial support and resources through the Home Stretch WA service program (eg. housing).
  • Providing tailored support services using a culturally safe and strengths-based approach and case coordination that is consistent with the Home Stretch WA Transition Coach Practice Principles.
  • Working collaboratively with relevant stakeholders including the Department of Communities’ Child Protection staff, foster families, carers, the young person’s kin, community and people identified as significant in their lives. 

Essential

We are looking for a passionate and motivated individual who is driven to deliver a quality service to young people. To be successful you will have the following skills and experience:

  • A qualification and/or relevant experience and skills in youth work or social services.
  • Ability to engage and build strong relationships with and motivate ‘at risk’ young people. 
  • A good understanding of the barriers and difficulties facing young people with experiences of out of home care.
  • Ability to be innovative and flexible, work independently, demonstrate initiative and work effectively under pressure.
  • Willingness to work across the local region on regular outreach; working in the clients home, in local community centres and any other venue where a client(s) may require support.
  • Ability to use electronic devices such as smart phone, tablet and computer and relevant applications.
  • Ability to assist Aboriginal young people to develop independent living skills.
  • Strong written and verbal communication skills.
  • Organisational skills in order to prioritise tasks and manage time effectively. 
  • Up to date knowledge of resources in the youth services field, including education, training and employment.
  • Demonstrated behaviour management, conflict resolution, advocacy and negotiation skills.

Please note: You will be required to hold a current C class Western Australian driver’s licence, WA Working with Children Check, and provide a national Police Certificate prior to commencing employment.
