About the role
The role of the Social Groups Support Worker is to assist in the running of 3Bridges groups, outings, and shopping trips. The role is responsible for delivering high quality of care and support to older people and people living with dementia. This can include supervision, providing practical assistance and personal care. The Social Groups Support Worker is to participate in all recreational activities and ensure clients are safe, engaged and always supported.
This role may require the provision of individual support to clients including in home visits, social outings, shopping, personal hygiene, domestic assistance, respite, and escorted transport.
The role aims to assist individuals in achieving and maintaining good quality of life, as well as feelings of belonging and self-fulfillment.
Several Permanent or Casual positions available.
- Contribute to the development of group programs and activities by providing input based on clients’ observations and feedback
- Maintain an understanding of client needs and use this knowledge when implementing recreational, social and/or educational activities for clients
- Encourage clients’ engagement in group conversations and activities
- Contribute to the assessment of clients’ needs by monitoring and reporting any changes in clients’ functional abilities
- Provide aid and support to clients to allow them to function more safely and independently
- Work collaboratively with the Team Leader to monitor and report changes/concerns regarding clients and identify new opportunities to service client needs
- This role may require the provision of individual support to clients including in home visits, social outings, shopping, personal hygiene, domestic assistance, respite, and escorted transport.
About You
- You have minimum 1 years’ experience in a similar role working with people who are frail, aged or have a disability
- You hold minimum Certificate 3 in Individualised Support/Aged Care or equivalent
- You are flexible to travel to different locations
- You have a sound understanding of Work Health & Safety standards
- You have a strong understanding of the New Aged Care Quality Standards
