CAREHUB DISABILITY PTY LTD is a new venture to facilitate the people with disabilities in our communities with desirable supports in their day to day life and community participation. We assist our participants in achieving a life of fulfilment and independence. Most of our participants are between the ages of 16 and 65, and as a disability support worker, you will experience the joy of helping them achieve their NDIS goals. Person-cantered approaches in daily activities are essential for empowering our participants.

About the Role:
• Assisting participants with important daily tasks.
• Ensuring daily personal hygiene and other personal care needs are met in a supportive and safe environment.
• Assisting with community access coordination and facilitation as part of a participant’s Support Plan.
• Facilitating participant community inclusion and networking opportunities.
• Collaborate with participants to achieve positive outcomes such as increased independence, social skills, personal development, higher quality of life, and enjoyment.
• Provide group and one-to-one support using a strengths-based and person-cantered approach.
• Assist participants with low and high support needs.
• This position would necessitate personal care and manual handling.
• Enabling participants to take ownership of their decisions and increase their independence
• Maintaining accurate records of participant progress notes and incident reports as needed.
Position Requirements:
- Understand the participant’s choice and be able to consult with their guardian as needed
- Be able to provide transportation in your own car, a hired car, or a work vehicle.
- Be able to plan enjoyable activities and experiences.
- Be able to handle complex or difficult situations rationally and calmly.
- Understanding of NDIS processes and the NDIS Quality and Safeguards Commission will be advantageous.
- Completion of a minimum Certificate 3 in Mental Health or Disability
- Current Australian Driver’s Licence – P2 minimum (essential).
- Meet the current COVID-19 Vaccine requirement – medical exemption acceptable (or willingness to obtain)
- Relevant DHS / NDIS checks and clearances in accordance with the role requirements (or willingness to obtain)
- Current First Aid Certificate (or willingness to obtain)
- Current Manual Handling Certificate (or willingness to obtain)
- Current Child Safe Environments Certificate (or willingness to obtain)
