We are trauma informed, therapeutic disability support services and provide services like Disability support work for NDIS and Non- NDIS participants, forensic disability support, specialist residential accommodation and also provide support to individuals to comply with conditions stipulated by the Adult Parole Board or the Forensic Leave Panel.
We are currently looking for an experienced and passionate Program coordinator to assist in growing the client base in Geelong. Having solid knowledge of the trauma informed support and environment and roll out strategies, this exciting position will have an existing portfolio of participants whilst actively influencing the uplift of MH&R holdings disability services.
- have the opportunity to make a real difference in people with disability
- supervise and run daily support and assistance services on a day-to-day basis, with living skills, connections with the community, personal and social development, employment, education, and training.
- assist participants to follow up actions from their case management plans including housing, income, education and training, health and wellbeing and general living skills and recreational activities; and
- complete client file notes as needed and assist with administration duties to help operations run smoothly.
- Attend care team meetings and networking and advocating for the participant under MH & R’s care to help them achieve their goals

- Provide trauma informed support.
Key selection Criteria:
- You have completed a relevant qualification in social sciences, human services, community studies, relevant experience and are looking forward to starting a role where you can have a real impact.
- You have demonstrated knowledge of person-focused practice frameworks such as strengths-based or trauma-informed practice.
- You have highly developed communication skills including relationship building, problem solving, report writing, and you pride yourself on your ability to motivate and advocate for clients to achieve outcomes.
- You would describe yourself as a team player and you enjoy interacting with a wide range of people.
- You are competent using the full suite of Microsoft applications and maintain accurate administrative records and progress notes.
- Experience in similar position or role is desirable.
- Strong and articulate communication skills
