Support Worker Job Description 

The Personal Care Worker’s  may include, monitoring and communicating consumer condition , transportation , house cleaning, personal hygiene , providing meals  and other health and related activities.

• Ability to follow the appropriate care according to the individual and personalised care plan. 
• Provide domestic help and personal hygiene tasks (showering ) Transport services to doctor’s appointments and shopping.
• Ability to write progress notes about client conditions with respect to client dignity, integrity, and confidentiality.
• Ability to follow the appropriate care according to the individual and personalised care plan.
•  Ensure care is provided in line with all relevant policies, procedures, and regulations
