About the role:

The Commonwealth Home Support Program (CHSP) Coordinator is to ensure the management of CHSP services to our clients, their families and carers using a wellness approach, whilst embracing a client centred methodology that demonstrates and promotes Amana Living’s Philosophy of Care and core values.

 Key Responsibilities

  • Conduct initial on boarding of assigned CHSP clients
  • Assist the Community Services Manager in identifying and developing new service delivery opportunities and apply for relevant funding where appropriate through the development and submission of funding applications 
  • Manage a small team of Community Care Providers
  • Provide support to clients, with the common goal being to enable clients and their carers to continue to live in the community.
  • Develop care plans for the provision of quality person centred support according to client needs, contractual obligations and policies and procedures. 
  • Conduct workplace assessments in accordance with Workplace Health and Safety guidelines to identify hazards and co-ordinate equipment requirements to ensure a safe work environment.
  • Ensure safe work practices are applied at all times and complying with the organisations WHS and infection control policies and procedures.
  • Lead by example to take responsibility for the identification, reporting, management and/or elimination of hazards and unsafe work practices.

Selection Criteria:

  • Demonstrated experience in a rostering role.
  • Demonstrated time management and organisational skills.
  • Proficient computer skills in Microsoft office suite and willingness to learn new programs.
  • Comfortable working in a small team environment and dealing with multiple work requests.
  • An understanding of issues and services for elderly people, persons with a disability and their families and carers.

Attributes for a successful candidate:

  • Knowledge of service provision, co-ordination, allocation and compliance with contractual obligations
  • Experience in people management 
  • Well-developed communication (written, verbal and listening) and interpersonal skills
  • Decision making and problem-solving skills to innovatively manage the provision of quality care.
  • A demonstrated empathetic and flexible approach to manage the sensitivities of client and carer issues.
  • Computer literacy in word processing, spreadsheets, and calendar management software
  • The ability to work autonomously with a team focus to contribute to business unit and organisational objectives.
  • A demonstrated understanding of health care within a community environment to co-ordinate quality service delivery.
  • Time management and organisational skills to effectively plan and prioritise work tasks.
  • Knowledge of manual handling principles with the ability to train others.
  • Basic First Aid Certificate.
  • At least 3 COVID Vaccinations 
  • Certificate III in Aged Care, Allied Health or equivalent field is desirable.
  • Solid case management experience
  • Experience creating Care Plans.
