Purpose of the Role
A Team Leader will undertake a commitment to Community Access Care (CAC) Pty Ltd.’s Vision, Mission and Values and promote the organisation’s ethos in all actions and activities to their position. The Team Leader is an integral part of the Service Delivery team. As an experienced member of the frontline team, you provide first level supervision and support to the team in line with the mission of the organisation to help them achieve their goals and the goals of the Participant. The key role of the Team Leader is to assist people with disabilities to improve the capacity and capability using a person-centred approach. This is achieved by supporting our participants to achieve their goals, build their skills and their independence.
Key Responsibilities of the Role
- Lead and motivate the team to achieve their goals;
- Maintain knowledge of our services and identify strategies to identify opportunities for business purposes;
- Assist in HR related processes including but not limited to recruitment, induction, training etc. as required;
- Provide feedback to the NDIS Business Manager where requested to complete Supervision;
- Phone calls/check in every 6 weeks with participants;
- Schedule face to face meetings every 3 months or sooner if required with participants;
- Mentoring and buddy shifts with Support Workers;
- Ensure the smooth running of SIL’s/Residentials/Programs;
- Ensure all appointments are booked and communicated and sent to rostering;
- Monthly checks where applicable for medication, bowel charts etc.;
- Maintain and review participant files monthly including but not limited to profiles, shift notes, activity plans etc. to ensure they are current;
- Contribute to monthly reporting requirements, data collection to assist in decision making;
- Provide feedback, where requested, to the NDIS Business Manager to assist in the completion of performance reviews;
- Set agenda’s, participate, facilitate and complete relevant actions plan follow-up for Team Leader Meetings and discussions, attend Stakeholder meetings as required; and
- Participate in the On-Call Roster including afterhours and on weekends.

- Knowledge and understanding of the National Disability Insurance Scheme (NDIS) and what this means to consumers;
- Ability to work in a team and autonomously; and
- Display commitment to evidence-based practice, continuous quality improvement, training, and professional development.
Selection Criteria
- Minimum Certificate IV in Individual/Disability Support, Community Services, Aged Care, working towards a qualification with a completion date of not more than 2 years when signing your employment contract with CAC or signing up for a relevant qualification within 1 month of signing your employment contract with CAC;
- Relevant certificate with relevant experience, or experience attained through previous appointments, services and/or study of an equivalent level of experience and/or experience to undertake the range of activities required for the role;
- Thorough knowledge of work activities performed within the workplace, sound knowledge of procedural/operational methods of the workplace;
- Experience in working with people with a disability will be highly regarded;
- Understanding rehabilitation and capacity building for and with people with a disability;
- Experience in direct participant support and care in the home;
- The ability to communicate clearly, professionally and respectfully with participants, families, colleagues, other agency personnel and external stakeholders;
- Assist with problem solving within your team e.g. rostering issues;
- Refined writing skill or be able to use program to assist e.g. talk and text; and
- Demonstrated computer skills and be able to use a smart phone or similar mobile devices.
