A new part time- contract opportunity has now become available for a Home Support Assessor to join our Sydney Metro Team, servicing clients in Western Sydney and Nepean regions.
This position will see you utilising your exceptional communication, interpersonal and time management skills, working independently with team support you will be visiting our clients in their own homes, conducting assessments, and reporting the outcomes. Your ability to be observant, intuitive and build instant rapport will assist in your success in this position.
Catholic Healthcare are one of the 13 organisations that successfully tendered to operate the My Aged Care Regional Assessment Services (RAS) commencing from 1 July 2015. RAS has been introduced by the Federal Government to streamline the assessment process for Older Australians seeking access to entry level support at home provided under the Commonwealth Home Support Programme (CHSP) and other available community supports.
About the Role
- Undertake face-to-face assessment of multiple Aged Care clients each day (as allocated).
- Update the electronic National Screening and Assessment Form (NSAF) on-line (real time) in conjunction with the client and generate a Support Plan with a reablement focus.
- Match and refer assessed clients to appropriate Commonwealth Home Support Programme Services (and other funded aged care services).
- Reassess existing clients where a significant change arises in the client’s circumstances or needs.
- Provide short-term case management as required to assist clients with high levels of vulnerability and multiple care needs to access a range of aged care and other services e.g. health, housing, disability, financial and aged care services.
- Apply competent computer skills and confidently utilise mobile technology.
- Work independently in this highly mobile role.
- Home Support Assessors will be expected to have local knowledge of services and the ability to build relationships with community aged care services within the regions
All successful applicants will be required to undertake comprehensive orientation and training upon appointment.
About you:
- The ideal candidate is a diligent, empathetic, and driven individual who is motivated by their passion to assist others who are vulnerable members of the community.
- Ability to communicate effectively and put clients at ease whilst building instant rapport
- Reside within the geographical area you are applying for and availability to travel throughout the region.
- Must have a current unrestricted Australian Driver’s Licence and registered and insured vehicle
- Relevant qualifications in Social Science, Nursing, Allied Health or related field is desirable.
- Tertiary qualifications in health or previous community experience in Aged Care will be highly regarded, but not essential.
- Computer literacy and reporting skills are essential.
